CCS Guidelines Knowledge to Action (KTA) Needs Assessment Survey

The CCS Guidelines Committee, in collaboration with researchers from Unity Health Toronto, request your participation in our CCS Guidelines Knowledge to Action (KTA) Needs Assessment.   

The survey is related to the latest CCS Guidelines on atrial fibrillation, heart failure, and dyslipidemia. Your input is crucial in shaping the direction of our Guidelines by informing us of the importance and relevance of our Guidelines, how helpful the KTA tools and resources are for you, and if there are certain barriers and facilitators that affect how you use and implement the Guidelines into your practice.
