Part 2 of the Pulmonary Arterial Hypertension (PAH) Education Series.
Presentations will feature case-based examples to provide an overview of the topic and include opportunities for interactive discussion and polling to emphasize the clinically relevant and practical points.
Learning Objectives:
- Apply evidence-based strategies for shared care with PH centers, incorporating interpretation of right heart catheterization findings to inform diagnosis, prognosis, and guide treatment decisions
- Coordinate effectively with PH centers and multidisciplinary teams to optimize care transitions and shared decision-making for patients diagnosed with PAH
- Participate in shared monitoring of individual treatment plans, manage therapy response, and address common comorbidities and complications in collaboration with PH centers
- Navigate advanced care pathways when therapies fail, including referral for transplant, palliative approaches, and end-of-life care planning
- Integrate emerging therapies and future innovations into long-term care planning, patient education, and follow-up
