CCS Secondary Prevention Pathway: Establishing standards for long-term post-acute coronary syndrome care

TThis webinar aims to describe the CCS Secondary Prevention Pathway to guide care following acute coronary syndrome (ACS) in Ontario, as well as explore opportunities for scale and spread of the pathway in regions across Canada. The session will cover the following learning objectives:

  1. Summarize the key components of guideline-directed secondary prevention care for patients after acute coronary syndrome (ACS).  
  2. Report known gaps in evidence-based post-ACS care in Canada. 
  3. Describe the CCS Secondary Prevention Pathway for post-ACS care. 
  4. Reflect on the extent to which evidence-based post-ACS care is available for patients in their region. 
  5. Determine if there is a need for the CCS Pathway in their practice setting as well as any challenges or barriers to its introduction.
